- A statement under penalty of perjury that you have a good faith belief the material was removed as a result of a mistake or misidentification
- Your name, address, telephone number, and email address
- A statement consenting to the jurisdiction of the appropriate court
Repeat Infringers
Appdorkar.com may, in appropriate circumstances, terminate access to users who are repeat infringers of intellectual property rights.
Good Faith Requirement
Please note that submitting false or misleading DMCA notices may result in legal consequences. Only submit a DMCA notice if you genuinely believe that your copyrighted work has been infringed.
